* Bring lab results, MRI/CT reports and CDs/digital imagining, Neuropsychological test results
* Bring updated Medication List to each appointment
*Request copy of Medication list from Assisted Living or Memory Unit Staff/RN and bring with you
*Be prepared to provide name and contact information for your preferred Pharmacy (including pharmacy being utilized by residential facilities)
*Arrive 15 min prior to appointment to allow for check in process
