How to prepare for your visits

* Activate your Patient Portal and utilize it frequently to schedule or reschedule follow up appointments, view Visit Summaries, send secure non-urgent questions to your care team, view your test results, access billing information, etc.
* Complete your Self Check-in online (7-10 days before your appointment you will receive a text invitation to Self-Check-in online).
* Upload results to Patient Portal or bring results: labs, MRI/CT reports, Neuropsychological evaluation report, etc.
*Arrive 15 min prior to appointment to allow for check in process to complete.
