Dr. Bach received her Medical Doctor degree in 1999 from Jagiellonski University Collegium Medicum, Krakow, Poland. She completed her Neurology residency and fellowship program at University of Wisconsin, Madison WI in 2006. She established a comprehensive Memory Clinic at Rockford Health System in 2011 and worked as neurologist focused on dementia care at Advocate Memory Center, Park Ridge, IL as well as at Northwest Neurology Ltd. Dr. Bach participated in Alzheimer's disease clinical trials and Alzheimer's disease biomarker studies. She is experienced in using newly approved Alzheimer's disease antiamyloid therapies, Alzheimer's disease plasma biomarkers and specialized dementia tasting such as Amyloid brain PET scan or skin biopsy.

In 2025. dr. Bach established Greenleaf Memory Clinic PLLC. She wanted to use her vast experience in dementia care to create a Dementia Hub for her local community, where she works and lives with her family (husband Mark and son Peter). Her goal is to provide continuity of care for dementia patients from diagnosis until transition of care or end of life.

Greenleaf Memory Clinic office is located at 355 Greenleaf St, suite H, Park City, IL, and is designed to provide comprehensive cognitive and neurological assessments, diagnostic testing, followed by personalized treatment plan, cognitive rehabilitation and caregiver education.

Dr. Bach would like to improve dementia care delivery by creating dementia community and involve dementia caregivers in clinic life. Experienced caregivers will have the opportunity to mentor new families and share their experiences and resources with others, as well as contribute to the content of this website.

Dr. Bach desires to expand dementia care by incorporating dementia prevention program called Power Your Brain. This program will utilize various methods i.e. diet, sound and sleep to lessen metabolic brain disturbances and restore energy delivery to the brain.
